My medical problem is older than the look back period for my travel insurance. Is it still considered pre-existing?

No, provided there has been no medical treatment or symptoms during the look back period.  The specific definitions for a pre-existing condition vary from policy to policy and can be found in the policy certificate. Most travel insurance policies have a look back period of 60 – 180 days.  For most travel insurance policies, a condition in not generally considered pre-exsiting if the condition is controlled soley through the use of medication, provided there has been no change to the medication during the look back period.  Travel medical plans have a much longer look back period, usually 1 – 3 years.
